READ CAREFULLY
WAIVER AND RELEASE OF LIABILITY
In consideration of Paintball Explosion furnishing services and/or equipment to enable me to participate in paintball, laser tag, airsoft and/or gel ball games, I agree as follows:

I who am eighteen years or older in age, fully understand and acknowledge that; (a) risks and dangers exist in my use of Paintball, Laser Tag, Airsoft and/or Gel Ball equipment and my participation in Paintball, Laser Tag, Airsoft and/or Gel Ball activities; (b) my participation in such activities and/or use of such equipment may result in my injury or illness including but not limited to bodily injury, disease strains, fractures, partial and/or total paralysis, eye injury, blindness, heat stroke, heart attack, death or other ailments that could cause serious disability; (c) these risks and dangers may be caused by the negligence of the owners, employees, officers or agents of Paintball Explosion; the negligence of the participants, the negligence of others, accidents, breaches of contract, the forces of nature or other causes. These risks and dangers may arise from foreseeable or unforeseeable causes; and (d) by my participation in these activities and/or use of equipment, I hereby assume all risks and dangers and all responsibility for any losses and/or damages, whether causes in whole or in part by the negligence or other conduct of the owners, agents, officers, employees of Paintball Explosion or by any other person.

I on behalf of myself, my personal representatives and my heirs, hereby voluntarily agree to release, waive, discharge, hold harmless, defend and indemnify Paintball Explosion and its owners, agents, officers and employees from any and all claims, actions or losses for bodily injury, property damage, wrongful death, loss of services or otherwise which may arise out of my use of Paintball, Laser Tag, Airsoft and/or Gel Ball equipment or my participation in Paintball, Laser Tag, Airsoft and/or Gel Ball activities. I specifically understand that I am releasing, discharging and waiving any claims or actions that I may have presently or in the future for the negligent acts or other conduct by the owners, agents, officers or employees of Paintball Explosion.
MEDICAL PERMISSION AUTHORIZATION
If the participant is of minority age, the undersigned parent or guardian hereby gives permission for Paintball Explosion to authorize emergency medical treatment as may be deemed necessary for the child named below while participating in paintball, laser tag, airsoft and/or gel ball games.
I HAVE READ THE ABOVE WAIVER AND RELEASE BY SIGNING IT AGREE IT IS MY INTENTION TO EXEMPT AND RELIEVE PAINTBALL EXPLOSION FROM LIABILITY FOR PERSONAL INJURY, PROPERTY DAMAGE OR WRONGFUL DEATH CAUSED BY NEGLIGENCE OR ANY OTHER CAUSE.
I FURTHER ACKNOWLEDGE that I am in the best position to determine my physical ability or the physical ability of the minor identified below to participate in the activities contemplated in this agreement, and acknowledge that I am in good physical and mental health, and not suffering from any condition, disease or disablement which would or could potentially affect participation in the activity.

By signing this form, you or your guardian takes full responsibility for the rental equipment that you check out. You assume the financial responsibility for any damage, theft, or loss to our equipment that you sign out. Your deposit will not be returned (state id) and/or your credit card will be charged for any damage, theft, of loss of our equipment. We will take legal action starting with but not limited to a Police report. Do not disassemble, remove, modify or alter any of our equipment for any reason. If you are experiencing any problems (no matter how small) please exchange the malfunctioning rental equipment for new equipment at our rental counter, where one of our employes will be happy to assist you.

All sales are final. Sorry, no refunds. No refunds - No exchanges on paintballs. Store credit only on unopened items (does not include paintballs). We appreciate your business. Hope you have a fantastic time.
